Better Outcomes, Greater Margins—Proven in 90 Days
EchoCarePlus offers a three‑month pilot designed to prove the value of pharmacist‑led Chronic Care Management without adding payroll or disrupting workflow.
During the Discovery Program, we:
Establish rapport with your clinic team and align with existing workflows.
Demonstrate our skill through pharmacist‑driven patient support and documentation.
Build enrollment by identifying patients who benefit most and tracking weekly progress.
Deliver measurable results with weekly summaries of patient enrollment, billing codes, estimated income, and care interventions.
This pilot gives clinics a risk‑free way to experience the benefits of CCM—improved patient outcomes, stronger financial performance, and reduced provider workload—before scaling to a full program.
Laying the Foundation
Program Introduction: Present the CCM Discovery Program to clinic staff and providers, outlining goals, benefits, and expectations.
Team Identification: Define the primary team members—both EchoCarePlus clinicians (pharmacist, nurse) and the clinic’s medical staff (PCP, office assistants)—who will lead the pilot.
Workflow Mapping: Establish communication protocols, patient concern triage pathways, and documentation standards to ensure seamless integration.
Trust Building: Reinforce the hybrid model by demonstrating in‑clinic presence and collaborative support, reducing skepticism about “outside” programs.
Kickoff Metrics: Set enrollment targets and reporting cadence (weekly summaries of patients, billing codes, estimated income).
Month One – Weeks 1–4: Building Momentum
Weekly Reports: Enrollment results, revenue, interventions, and care plan evaluation/feedback.
Communication Review: Evaluate provider/staff communication and refine as needed.
Care Plan Development: Pharmacist‑led medication reconciliation, preventative service documentation, and patient education.
Interventions: Lifestyle tracking, adherence support, and motivational interviewing to build trust.
Months Two & Three – Optimization and Integration
Follow‑Up Goals: Select specific gaps in care to close and quality metrics to address.
ICD‑10 Review: Evaluate and update codes to ensure accuracy and maximize reimbursement.
Recommendations: EchoCarePlus team provides guidance; clinic manager/PCP gives direction.
Workflow Management: Weekly reports continue, with seamless integration and ongoing refinement.
Wrap‑Up & Transition
Final Evaluation: Enrollment targets, care gaps closed, STAR/HEDIS metrics improved.
Comprehensive Reports: Summaries of patient enrollment, billing codes, income estimates, and interventions.
Next Steps: Transition to full CCM program, with optional expansion into RPM and BHI.
