Who should get CCM?

Primary care providers now operate within strict time limits during office visits, often having to react to acute concerns before in-depth chronic condition evaluation and lifestyle modification changes. Following up on medication changes to identify adverse effects or adherence issues and revealing compliance concerns with new and existing provider-directed health goals is not always possible at the level preferred by the provider.
Patients who are eligible for Medicare and have two or more chronic diseases expected to last more than a year are eligible for the CCM program. In many cases, the additional support this program provides is at no cost to the individual.
